Dpcumentary Requirements for US Applicants
- Current comprehensive resume with 3 (2x2) signed photo
- Transcript of Records (TOR) & Relevant Learning Experience (RLE) - 2 sets of certified copy of the same in sealed envelopes from the school
- Diploma (copy) – high school & college
- Board Rating (copy)
- Board Certificate (copy)
- Employment Certificate (most recent – orginal & copy)
- PRC License(current)
- Nursing Training Certificates related to area of practice (at least 5)
- 2 Letters of Reference (Nursing Director & Nursing Supervisor)
- Passport (copy)
- Birth Certificate (clear copy from Municipal Office)
- Marriage Certificate (clear copy from Municipal Office) – if married
- Name Change Document – if applicable
- IF AVAILABLE:
- CGFNS or NCLEX-RN Certificate
- TOEFL/TSE Bubble Form or IELTS Certificate or MELAB Certificate

Nurses for the USA - Must have passed the Philippine Nursing Board exam with a rating of 80% and above and the CGNFS exam. For applicants in Metro Manila and Regions 3 & 4, please apply in person with the following: updated Resume, TOR/ RLE, Diploma, Board Rating, Board Certificate, Certificate of Employment, Professional License, seminar certificates, Passport , NBI clearance, Birth Certificate, Marriage Certificate, 4 passport size pictures, two original references.
